Denied by insurance? A denial is not the end. Roughly 60–75% of prior-authorization denials get overturned at the first internal appeal, and 80%+ at external review. Appeal the exact reason printed on the denial letter, file within 180 days, and have your prescriber attach BMI records, comorbidity diagnosis codes, and any prior failed weight-loss attempts. Read this before you read the list Not being on this list does not mean a seller is safe. There is no verified-safe seller in this market. The FDA has said the entire category is an enforcement priority, three waves of warning letters went out in twelve months, and companies here vanish without notice holding customer money. A clean record often just means nobody has looked yet.

Founder sentenced to 70 months in federal prison plus $5M forfeiture for selling mislabeled compounds. The case involved forged certificates — the COA paperwork itself was fake. This is the answer to “but they post lab results.”

FDA raided the warehouse after the company ignored prior warning letters and marketed research-labeled product for human consumption. Operation ended.

The FDA found semaglutide, retatrutide, cagrilintide, tirzepatide and mazdutide sold as “research” products — while the company's own Facebook page promoted “Enhanced Weight Loss” from combining cagrilintide and semaglutide. The research label and the marketing contradicted each other in public.

All six sued by Eli Lilly on the same day for selling black-market retatrutide — a drug still in Phase 3 trials and approved nowhere on earth. Lone Star, Texas Peptides and Aesthetic Envy pulled their retatrutide pages once the suits landed. Lilly says samples it tested ran from 0% active ingredient to overdose strength, and some contained entirely different drugs including insulin.

Altru Telehealth · Momentum Health 360 High · FDA — false facility claims 2026-06-08

Both cited by the FDA for false facility claims and implied FDA approval, Momentum also for branding misrepresentation. Letters MARCS-CMS 728274 and 728286. Worth sitting with: “fake credentials” is not a suspicion in this market, it's an enumerated FDA violation category with companies named under it.

Both cited for equivalence claims, implied FDA approval, and branding misrepresentation. Letters MARCS-CMS 721455 and 717986.

Triggered Brand High · Connecticut AG lawsuit 2025-05-21

Sued by Connecticut's attorney general for selling “research grade” GLP-1s straight to consumers with no prescription or medical oversight. Nominally a lab supplier — but its own social media and website targeted people looking for weight-loss injections. Alleged unlicensed pharmacy practice, up to $5,000 per violation.

Made-in-China.com High · platform-level shutdown 2026

The trade platform agreed to cease all sales of unlawful “research grade” GLP-1s into the United States following Connecticut AG action. Not one seller — a whole pipeline.

Mochi Health Caution · manufacturer lawsuit 2025-04

Lilly's suit alleges the telehealth company changed patient doses en masse — batch protocol changes five times in eight months — without individual consultation, which contradicts the “personalized dosing” legal theory its compounded product depends on. Lanham Act and California claims allowed to proceed April 2026.

Henry Meds Caution · lawsuit, review integrity 2025-04

Sued by Eli Lilly over compounded GLP-1 sales after the shortage ended. Separately: Trustpilot has flagged its profile for suspected fake reviews while it displayed 4.5 stars, and the BBB rates it F with 227 complaints. Its advertised “$197/mo” requires a 12-month prepay.

Hims & Hers Caution · FTC action, billing 2026-07-29

FTC plus California and Utah sued over charging without express informed consent, obstructed cancellation, and sharing health data with ad platforms. Novo Nordisk had already ended its partnership in 2025, accusing the company of mass compounding “under the false guise of personalization.” Billing and marketing conduct — not proven product contamination.

This is not a summary or a sample — it is pulled directly from the FDA's own warning-letter database export, and every letter is published individually on fda.gov where you can read it in full. A warning letter is a formal notice of violation carrying a 15-day response deadline; it is not itself a penalty, but failing to answer one can lead to seizure, injunction, civil penalties, or prosecution.

The shape of it is worth seeing: 110 of the 165 are telehealth companies cited for false or misleading claims — not shady overseas labs, but slick consumer-facing brands. Another 12 were cited for selling unapproved drugs over the internet, 8 are compounding pharmacies with adulterated products, and 7 are manufacturers with outright manufacturing violations.

Two tricks worth knowing before you read any product page

They rename the drug so you don't recognize it The FDA's letter to Gram Peptides documents retatrutide being sold as “GLP-1-R peptide” and tirzepatide as “GLP-2 peptide.” Pink Pony Peptides listed “GLP-2 TZ” and “GLP-3 RT.” Someone ordering “GLP-3 RT” may never register that they just bought retatrutide — a drug that has never passed a safety review anywhere on earth. The code name is the whole point, and it is now standardized across unrelated sellers, so buyers meet it as if it were ordinary industry nomenclature.

Bundled bac water is what proves the label is fiction The FDA cited Prime Sciences and Mile High Compounds for selling bacteriostatic water and syringes alongside their peptides. Because the bundle demonstrates intent to inject, the agency held that the bac water itself becomes an unapproved drug in that context. Bacteriostatic water is perfectly legal on its own — sold as part of an injection kit next to “research only” vials, it is the evidence that the research label was never real.

What the forums are saying — and why it sits in its own box

Everything in this box is an allegation, not a finding. These come from bodybuilding forums and review sites, not from regulators, courts, or labs. Some review sites earn affiliate money for steering readers elsewhere, and forum posts are sometimes competitor sabotage. They are worth knowing as signals to check yourself — never as verdicts. A vendor missing from this box has not been cleared; a vendor in it has not been convicted.

Reviewers accuse the vendor of photoshopped test reports. If true, that's the same forged-COA pattern that sent the Paradigm Peptides founder to prison — and it's exactly why you verify a certificate through the lab's own QR or task-ID lookup rather than trusting the PDF you were handed.

Inside the vials

Not speculation. These are published findings from testing programs and from the manufacturer's own analysis of black-market product.

Lead and mercury

A 2026 investigation sent ten peptides to a lab. Pharmaceutical-grade product came back at 98–99%+ purity, endotoxin under 0.5 EU/mg, no heavy metals. Lower-tier research-grade product averaged 75% purity, 8+ EU/mg endotoxin, and contained lead and mercury.

Bacterial endotoxin — a 17× spread between quality tiers

Endotoxin causes fever, inflammation, and at worst septic shock. It survives sterilization, and it is completely invisible on a purity certificate. One community testing program found an 8% endotoxin positivity rate across submitted samples.

Entirely the wrong molecule

One tested “semaglutide” sample was an unknown compound at 2,847 daltons. Real semaglutide is 4,111. Eli Lilly reports black-market samples ranging from zero active ingredient to overdose strength, some containing different drugs including insulin — which can kill someone who isn't expecting it.

People are already in hospital from the dose math

The FDA reports multiple adverse events, some requiring hospitalization, tied to dosing errors with compounded injectable semaglutide — patients measuring and self-administering the wrong dose, and in some cases clinicians miscalculating it. This is the part no certificate addresses: even authentic product in a vial you draw yourself puts the arithmetic on you.

Five things that sound like proof and aren't

Research use only means it's a legit grade of product
It's a legal disclaimer, not a quality tier. It exists so the seller can argue they never sold a drug for human use. It guarantees nothing about the vial, and both the FDA and the courts have rejected it as a shield when the marketing points at consumers.
They post a COA, so it's tested
A purity certificate proves two things about a sample the vendor chose to send: the molecule's identity and its peptide-fraction purity. It does not test how many milligrams are in your vial — a 99% pure vial can be half-dosed — and it says nothing about sterility, endotoxin, heavy metals, or residual solvents. It also cannot prove your vial came from the batch that was tested. Purity is not safety.
FDA registered facility
Facility registration is a form any manufacturer files. The FDA does not inspect, endorse, or approve products because of it, and explicitly forbids using registration to imply approval. Treat this badge as a red flag, not a credential.
They have thousands of five-star reviews
Trustpilot flagged one major seller's profile for suspected fake reviews while it displayed 4.5 stars and held an F from the BBB with 227 complaints. Review scores in this market are a marketing surface.
They're selling retatrutide, so it must be available now
Retatrutide is approved nowhere on earth. Lilly won't even file it with the FDA until early 2027 — realistic pharmacy availability is 2028. Anyone selling it today is by definition selling an unregulated product of unknown contents. Same goes for cagrilintide and for “generic semaglutide”: no FDA-approved generic exists, and US patents run to December 2031.

Part four and a half · The question everyone asks

Are the certificates real?

Some are. Some are outright forgeries. You cannot tell by looking at one — but you can verify a real one in about sixty seconds, and almost nobody ever does.

Forgery is documented, not theoretical The founder of Paradigm Peptides went to federal prison for 70 months in July 2026 with admissions that he had been forging laboratory certificates. Testing labs report catching forged COAs built from stolen lab logos, fabricated signatures, lifted PDF headers, and digitally manipulated chromatograms. And in the FDA's June 2026 letters, “false facility claims” is a formal citation category — with companies named under it.

What a forged or worthless certificate looks like

And the hard ceiling, even on a genuine certificate: it covers identity and purity of a sample the vendor chose to send. It never covers milligram quantity, sterility, endotoxin, or heavy metals. A perfectly authentic COA and a half-dosed, endotoxin-positive vial can both be true at the same time. The only way to know what's in a specific vial you own is to send one from your own batch for quantification testing yourself.
